3. Membership and Fees
To access our services, patients must enroll in one of our membership plans. Membership details, including fees and covered services, are outlined on our Membership page. Membership fees are subject to change, and we will provide at least 30 days’ notice before any changes take effect.
4. Non-Participation in Insurance
OBMC operates on a direct primary care model and does not participate in any health insurance or HMO plans. Our services are not a substitute for health insurance. We recommend that patients maintain separate health insurance policies to cover services not provided by OBMC, such as hospitalizations, specialist consultations, and emergency care.
5. Cancellation and Termination
Patients may cancel their membership at any time by providing a 30-day written notice. Upon cancellation, membership will continue until the end of the current billing period. OBMC reserves the right to terminate a patient’s membership with a 30-day written notice for reasons including, but not limited to, non-payment of fees, abusive behavior, or failure to adhere to recommended treatment plans.
